Position: Patient Care Coordinator (ED Position)
Location: Bradenton, FL

Job Summary:
* Work closely with Manatee Memorial Registrar to update demographic for identified MCR patients to capture up to date information and update EHR records. * Effectively communicate with Hospitalist and various departments within the hospital and having the knowledge of various diagnoses. * Able to navigate multiple healthcare systems and upload hospital records as well as missing information in patients’ chart. * Responsible for notifying Primary Care Provider of ED admission and or discharge. * Assist with scheduling appointments. * Determine an appropriate ED visit versus Primary Care Provider visit. * Identify needs, barriers and initiate appropriate referrals to internally and externally. * Assist patient and the family with navigation of their health care system. * Document all encounters and patient related discussions, telephonic or in-person, in the patient medical record per organization. * Responsible for creating and managing reports. * Assist patient and family to access available community resources as appropriate. * Maintain contact with patient and family through transitions of care while coordinating with other care team members. * Provides coverage for Care Coordinator colleagues as assigned. * Participates as an active member of the patients’ care team, collaborating with patient, family, providers, site staff and other care partners. * Maintains accurate records regarding case load, stratification, and frequency of contact. * Participates in departmental, site, and other meetings as assigned. * Supports the goals and mission of MCRHS, Inc. * Adheres to all safety policies and procedures. * Maintains a clean, clutter free and well organized workspace. * Participates in community functions as appropriate and promotes the concepts of “Patients First” and “PCMH”. * Performs and embraces all other duties as assigned. * Maintains a caseload consistent with program guidelines. *Maintains accurate travel documentation. * Performs other duties as assigned.
Requirements:
  • Graduation from an accredited university with a background in science, including a BA or BS in Public Health, Behavioral Science or similar degree; or a Licensed Nurse with a minimum of 5 years of clinical experience in both inpatient and ambulatory care settings in provision of direct services to patients preferred.
  • A minimum of 3 years of experience in care coordination, case management, or related area may substitute for the educational requirement above.
  • Experience in Chronic Care Management desirable
  • Case Management or related Certification desirable
  • Proficient in Microsoft Office products, EPM, and EHR software preferred.
  • • Possession of a current Florida State driver’s license and a registered, insured vehicle as periodic travel may be required to effectively service clients.
  • Must maintain a professional demeanor and appearance at all time
  • Must be able to handle multiple projects simultaneously and set priorities
  • Must possess a current valid driver’s license for the State of Florida MCR Health Services is a drug free workplace. All job applicants selected for employment are required to submit to a pre-employment drug test.
